CSAC Hardship Waivers

To expedite the processes of reviewing and ruling on a medical hardship waiver, you are asked to send the following information on the student-athlete and the team they were a member of:

  1. Medical documentation from a physician; not a trainer; documenting the nature of the illness/injury and establishing the student-athlete’s inability to compete for the remainder of the season as a result of that injury or illness. This documentation must be on the physician’s letterhead and have his/her signature.
  1. The competition schedule of the team that the student-athlete was a member of.
  1. A roster of the team.
  1. Box scores/statistics for EVERY GAME of the season (the cumulative statistics for the season do not supply the information needed to rule on the hardship.) This is necessary for the determination of the number of contests the student-athlete competed in, the time frame of when the injury occurred (first ½ of the season) and helps certify that the student-athlete did not come back at some point later in the season to compete.
     
  2. Ethical Conduct Certification letter requesting the medical hardship waiver signed by both the AD and SA.

Failure to send the above information will delay any rulings until the required information is forwarded to the conference office.
